IT Change Management minimizes risks and disruptions by following a structured process for implementing changes in IT systems. It improves efficiency, enhances user adoption, and ensures better resource allocation. Overall, it maintains the stability and reliability of IT operations while supporting organizational goals.

Procedure
- Title: Enter a descriptive title using action words (ex. Update, Move, Decommission, etc.)
- Requester: Enter the name of person completing the work.
- Change Type: Select Normal if the work will be completed after the next Change meeting. Select Expedited if the work must be completed prior to the next Change meeting.
- Change Sponsor: Enter the name of the person who requested this work be done.
- Change Start Time: Enter the Date and Time of when the work will start.
- Change End time: Enter the date and Time of when the work is expected to be completed.
- Summary of Work: Enter a brief description/summary of what work/change will occur. This should be descriptive but only a summary

- List of Work: Enter the details of the work that will be completed during this change.
- Test Validation: Enter the details of how it was tested to verify the impact of the change and how it will be tested to confirm the status of the change.
- Impacted Assets or Services: Enter the impacted hardware, software, and services due to the requested change.
- Justification/Goal and Benefit of Change: Enter the reason why this change is needed and what the goal and benefit will be of the change.
- Rollback Plan: Enter details on what will be done to revert back to pre-change status if the change is not successful.

- Impact on Academic Calendar: Enter any details that the change may have on the academic calendar (impact to registration, courses, interactions with students, etc.)
- Notification Plan: Enter the details of who will be notified, what the notification will state and who will complete the notification.
- Attachments: Upload any documentation that should be included in the request.
- Once implemented, the requestor will go into the form, update the Change Outcome, and then mark the Implement Change task as completed
