Dynamic Forms - Access to Organization

Introduction

This service request is used to request access to an existing organization within Dynamic Forms.  This is necessary for employees who need access to the administrative portal for processing submitted forms or designing new electronic forms.

Procedure

To request access to an existing form, select the Request Access button.  You will be asked to identify the supervisor/owner of the organization you are requesting access to, the name of the organizations, and the type of access.

If requesting access for multiple people, please include the full names and CCBC email addresses in the "Organization and Access Type" field.

Organizations is the term used within Dynamic Forms for departments.  Each organization has three instances - Production, Development, and QA.  An example of a production organization is "CCBC - Financial Aid".   Only Form Designers need access to the Development and QA instances.

Access Types include:

  • Form Reviewer - only need the ability to review and process forms.
  • Form Designer - ability to review, process, build, and maintain forms.  Form Designer requests require supervisor/organization owner approval.

Requirements

Employees requesting access to an organization should know the name of the supervisor/organization owner, the name of the organization, and the type of access being requested.  Employees requesting the Form Designer access type will require supervisor/organization owner approval.
